When Dorothy returns to the house during the tornado, she tries to get into the cellar but the door is bolted and they can't hear her.  She is carrying Toto and the picnic basket.  She goes inside and the next frames show her in her bedroom with Toto but no picnic basket.  Where did she put it!

When Dorothy is up inside the cyclone, she sees Miss Gulch riding her bicycle, who then turns into a witch on a broomstick.  When the house lands, it lands on the witch, and the Munchkins sing about how "the witch to satisfy an itch when riding on her broomstick thumbing for a hitch", so the house landed on her.  So the witch who was killed by the house was Miss Gulch/Witch of the East, and it's her sister that is the Witch of the West.
